How to create a feedback form ?

 Hello evryone,

This week my tutor show to me how to create a feedback form.Today I will show you how I made mine form.

In a feedback form you can write questions and then, you will receive feedback to improve your work.

In this link you will find this options.

 I choose the blank option.


Also, there are diferent types of questions such as a paragraph or multiple choice.

I put seven questions and I named this form feedback and user experience.

    It is okay that you have shown how you have created the feedback form but really what you should be showing is the feedback you received and your response to this. Please make sure you add this to your blog.
